Best Cushioned Running Shoes for Runners With Knee Pain
The honest version of this guide has to start by undermining its own premise.
The uncomfortable finding
If you search this topic you will be told, repeatedly and confidently, that softer shoes protect your knees. The research does not support that claim nearly as strongly as the marketing does.
The most direct evidence comes from a double-blind randomised controlled trial that gave 247 runners shoes identical in every respect except midsole hardness — soft or hard — and followed them for five months. Injury rates did not differ between the groups. Midsole hardness, in that trial, simply did not predict who got hurt.
It gets stranger. Several biomechanics studies have found that runners in highly cushioned shoes show increased leg stiffness and greater impact loading, not less — the body appears to adjust its landing to the surface it expects, a phenomenon sometimes described as impact anticipation or muscle tuning. Softer underfoot does not reliably mean gentler on the joint, because you are not a passive mass being dropped onto the road. You adapt.
What this means for you. Cushioning is a comfort and durability decision far more than it is a knee-protection decision. Buying a maximalist shoe because you have knee pain is not a well-supported strategy. Buying one because it feels better over 90 minutes and lets you keep running is a perfectly good reason — it's just a different reason.
So what does the evidence actually favour?
The most consistent finding in footwear research is unglamorous: comfort predicts outcomes better than any single construction feature. Runners who select shoes on subjective comfort tend to sustain fewer problems than those matched to shoes by arch height, pronation category or cushioning level. The "comfort filter" idea — that people self-select toward the mechanics their body prefers — has held up better than the prescriptive models that dominated shoe retail for thirty years.
That's genuinely useful, because it means you are a better judge than a shop's gait treadmill. It also means most of the categories used to sell shoes to you are weaker than they sound.
Things with reasonable support
- Comfort on your own feet, over a realistic distance. Ten minutes on a treadmill is not a test. Most retailers with a returns window are effectively lending you a proper trial.
- Cadence, not footwear. Increasing step rate by around 5–10% reduces load at the knee fairly reliably across studies — one of the few interventions with consistent support. It costs nothing.
- Loading progression. Sudden increases in volume or intensity predict injury far better than shoe choice does.
- Strength work. For patellofemoral pain specifically, hip and quadriceps strengthening has meaningful evidence behind it. A shoe does not.
Things sold harder than the evidence justifies
- Motion-control shoes prescribed by pronation category.
- Softer midsoles as an injury-prevention measure.
- Carbon plates for recreational runners with joint pain — designed for economy at speed, not comfort at easy pace.
What to look for if you're buying anyway
You still need shoes. Here are the characteristics that are defensible for a masters runner managing knee discomfort, in order of how much they matter.

The single highest-value action in this guide is not a purchase. It's rotating between two different shoe models rather than wearing one pair into the ground, and raising your cadence slightly. Both are better supported than any specific shoe recommendation anyone can give you.

When a shoe is the wrong tool entirely
Some knee pain is a footwear-and-load problem. Some is not. Treat these as reasons to see a physiotherapist or doctor rather than to keep shopping:
- Pain that persists at rest or wakes you at night.
- Swelling, locking, giving way, or a knee that catches.
- Pain that has not improved after four weeks of reduced load.
- Any pain following a specific traumatic incident.
Buying a new pair of shoes when one of those is true mostly buys delay, and delay is the expensive part.
